Virtual Collections Enrollment for Recovery & Professional Wellness

I approve a one-time PPW set-up/origination fee of $20 to be applied at the time of enrollment. The fee will be deducted from the credit card on file with PPW.

I approve and give permission for the fee of $35 to be deducted prior to each collection. The fee will be deducted from the credit card on file with PPW. If the collection goes over the allotted 30 minutes you may be charged an additional fee.

I hereby give permission for PPW to contact me and record specimen collection through a video meeting service(s), e.g., Zoom.

I will be responsible for the collection of my own specimen (e.g., hair, nail, blood spot) while on a recorded live-stream service with an PPW collector witnessing the entire process to document control and custody protocols.

I understand that if at any time during the collection process I step out of frame, my hands are not visible or connection is lost, the collection and all fees will be forfeit, reported to my program, rescheduled and possibly deemed “NO SHOW.”

I will make every effort to obey all safety precautions as listed in writing and as explained by the PPW collector. At any time, I may address concerns or ask questions of the PPW collector.

Please give a three-hour notice of cancellation. Cancellations within the three-hour period, or no shows, will be charged the collection fee of $35. If you are more than fifteen minutes late you will be charged the $35 collection fee and rescheduled.
